This travel notice submission impacts the debit card indicated above only. If you have a joint owner on your account with a debit card, a form will need to be submitted for their card as well.

Please note if you’d like to add both joint owners and beneficiaries you will need to complete this form twice.

You each will be sent a change-in-account membership agreement via DocuSign to fill and sign. Your new joint owner will also need to complete an ID-Pal verification. You will receive your ID-Pal link.

This form is to add a joint owner to your existing membership account. If you wish to open a new account with a joint owner, please submit a new application on our website.

You will be sent a change-in-account membership card to sign before we can make the requested changes to your account. If you have a joint owner, they will need to sign as well. You will receive your DocuSign within 1-2 business days of submitting this form.

Billing/Shipping Details:

• The cost for 60 standard checks varies and is typically around $28, additional shipping fees and state taxes may apply depending on your location.
• Check orders will be billed to your Clean Energy CU checking account, funds will be deducted from your account within 14 days of ordering. Sufficient funds must be available for check orders to be processed.
• All checks must be shipped to the address we have on file for your account.
• Once the check order is placed you will receive a tracking email to the email that we have on file.
